Overview

Macular oedema developing in the weeks after cataract surgery, characteristically as vision that improved and then fell away again. The front of the eye can look calmer than the drop in central vision implies, and OCT shows cystoid change where it is available. The timing is the clue, and an eye that is also painful and red in the same window is a different and more urgent problem.

Recognition

Symptoms: what patients report

  • Reduced central vision after cataract surgery.
  • Blurred vision after an initial period of improvement.
  • Post-cataract timing rather than immediate postoperative pain is typical.

Signs: what the examination shows

  • Reduced central visual acuity compared with expected postoperative recovery.
  • Macular thickening or cystoid change on OCT where available.
  • Anterior segment may appear quieter than the degree of central blur would suggest.
