10905·Referred Comprehensive Initial Consultation

10907·Comprehensive Initial – Previously Attended Another Practice

Professional attendance >15 min, first in a course of attention, if the patient has attended another optometrist for a service to which item 10905, 10910, 10911, 10913, 10914 or 10915 applies — within 36 months (age <65) or 12 months (age ≥65).

Patient seen at another practice within the applicable period. Reduced fee item.

10910·Comprehensive Initial Consultation – Patient Under 65

Professional attendance >15 min, first in a course of attention, for a patient under 65 years of age who has not received a service to which item 10905, 10907, 10913, 10914 or 10915 applies within the previous 36 months.

Patient must be under 65 and not have had an eligible initial claim at any optometrist within 36 months.

10911·Comprehensive Initial Consultation – Patient 65 or Older

Professional attendance >15 min, first in a course of attention, for a patient aged 65 or older who has not received a service to which item 10905, 10907, 10910, 10913, 10914 or 10915 applies within the previous 12 months.

Patient must be 65+ and not have had an eligible initial claim at any optometrist within 12 months.

10913·Comprehensive Reassessment – Significant Change or New Signs/Symptoms

Professional attendance >15 min, first in a new course of attention, if the patient has suffered a significant change of visual function or has new signs or symptoms, unrelated to the earlier course of attention, requiring comprehensive reassessment.

Significant change in visual function OR new signs/symptoms. Within 36 months (<65 yrs) or 12 months (≥65 yrs) of a prior initial consult.

10914·Comprehensive Reassessment – Progressive Disorder

10915·Diabetic Eye Examination (with Dilation)

10916·Brief Initial Consultation (≤15 min)

Professional attendance, being the first in a course of attention, of not more than 15 minutes duration. Not applicable when co-claimed with items 10938, 10939, 10940, 10941, 10942 or 10943.

First visit in a new course of attention. Duration ≤15 minutes. Cannot be co-claimed with visual fields or specialised items.

10918·Subsequent Consultation

Professional attendance, being the second or subsequent in a course of attention, unrelated to the prescription and fitting of contact lenses. Not applicable when associated with computerised perimetry items 10940 or 10941.

Second or later visit in the same course of attention. Not for contact lens fitting visits.

Contact Lenses

10921·Contact Lens Fitting – High Refractive Error

Fee $198.50·85% $168.75

All professional attendances after the first, regarded as a single service, in a course of attention involving prescription and fitting of contact lenses — for patients with myopia ≥5.0D, hyperopia ≥5.0D, astigmatism ≥3.0D, or anisometropia ≥3.0D in at least one eye.

High refractive error threshold must be met. Eligible once per 36 months per qualifying condition. Delivery no longer required as of 1 March 2025.
Once per 36 months for each qualifying condition.

10924·Contact Lens Fitting – Irregular Astigmatism

Fee $250.40·85% $212.85

All professional attendances after the first, as a single service, in a course of attention involving contact lens prescription and fitting for patients with irregular astigmatism confirmed by keratometry/topography — where BCVA with spectacles is worse than 0.3 logMAR (6/12) and a contact lens would improve it by ≥0.1 logMAR.

Irregular astigmatism confirmed by keratometry or topography. BCVA <6/12 with spectacles. CL improves acuity by ≥1 line. Once per 36 months.

10926·Contact Lens Fitting – Telescopic System

All professional attendances after the first, as a single service, in a course of attention involving contact lens fitting for patients with corrected VA of 0.7 logMAR (6/30) or worse in both eyes, for whom a contact lens is prescribed as part of a telescopic system.

Corrected VA ≤6/30 in BOTH eyes. Contact lens is part of a telescope (bioptic/contact lens telescope). Once per 36 months.

10927·Contact Lens Fitting – Opaque Lens for Dazzle/Diplopia

All professional attendances after the first, as a single service, in a course of attention involving prescription and fitting of a wholly or segmentally opaque contact lens for alleviation of dazzle, distortion or diplopia caused by pathological mydriasis, aniridia, coloboma of the iris, pupillary malformation, significant ocular deformity, or corneal opacity.

Opaque or semi-opaque CL for specific iris/pupil/corneal abnormalities causing dazzle/diplopia. Once per 36 months.

10928·Contact Lens Fitting – Physical Deformity (Unable to Wear Spectacles)

All professional attendances after the first, as a single service, in a course of attention involving contact lens fitting for patients who, because of physical deformity, are unable to wear spectacles.

Physical deformity must make spectacle wear impossible. Not cosmetic/preference. Once per 36 months.

10929·Contact Lens Fitting – Other Medical/Optical Condition

Medical/optical indication must be documented on the account. Not for lifestyle/sporting/cosmetic. Once per 36 months.

10930·Contact Lens – Material or Design Change

All professional attendances as a single service in a course of attention involving CL fitting where the patient already meets criteria for an item in range 10921–10929 and requires a change in contact lens material or basic lens parameters (not a simple power change) due to a structural or functional change in the eye or an allergic response — within 36 months of the original fitting.

10938·Computerised Perimetry – Glaucoma High Progression Risk (Bilateral, 3rd Test)

Full quantitative computerised perimetry (automated absolute static threshold), bilateral assessment and report — for glaucoma patients with a high risk of clinically significant progression who have already received two perimetry services (10940 or 10941) in the previous 12 months.

NEW from 1 March 2025. Third perimetry test in 12 months — only for high-risk glaucoma progression. Max once (incl. 10939) per 12 months.

10941·Computerised Perimetry – Unilateral (Up to 2 per Year)

Fee $46.00·85% $39.10

Full quantitative computerised perimetry (automated absolute static threshold), unilateral assessment and report — indicated by relevant ocular disease or suspected pathology of the visual pathways or brain. Not multifocal multichannel. Performed by optometrist. Patient has fewer than 2 perimetry services (10940/10941) in the current 12-month period.

Specialised

10942·Low Vision Assessment

Fee $40.00·85% $34.00

Testing of residual vision to provide optimum visual performance for a patient who has best corrected VA of 6/15 or N12 or worse at 40cm in the better eye, or a horizontal visual field of less than 110 degrees. May include spectacle correction, contrast sensitivity, glare sensitivity, or magnification aid prescription.

BCVA ≤6/15 (or N12 at 40cm) in better eye, or visual field <110° horizontal. Maximum 2 per 12 months. Cannot co-claim with CL items 10916 or 10921–10930.
Maximum twice per patient in a 12-month period.

10943·Children's Vision Assessment – Binocular/Accommodative Dysfunction

Fee $40.00·85% $34.00

Additional testing to confirm diagnosis of, or establish a treatment regime for, a significant binocular or accommodative dysfunction in a patient aged 3–14 years. Includes assessment of accommodation, ocular motility, vergences, fusional reserves, or cycloplegic refraction. Not for assessment of learning difficulties.

Patient must be aged 3–14 years. Once per 12 months. Not for learning disability assessment. Can co-claim with 10910, 10913, 10914, 10915, 10918.
Once per patient in a 12-month period.

Procedures

10944·Removal of Embedded Corneal Foreign Body

Fee $86.35·85% $73.40

Complete removal of an embedded foreign body (including rust ring, if present) from the cornea — not more than once on the same day by the same optometrist, excluding aftercare. Claimable when fully removed, or when patient is referred to an ophthalmologist after partial removal at second attendance.

Once per day per practitioner. Must be completely removed OR patient referred to ophthalmologist after second partial-removal attempt.

Domiciliary

10931·Domiciliary Visit – Flag Fall

Fee $45.95·85% $39.10

Flag fall item applicable once per visit to a domiciliary location (home visit, residential aged care facility, or institution) and billable only for the first patient seen at that location on that occasion, regardless of number of patients seen.

NEW from 1 March 2025. Once per visit location, first patient only. Must be co-claimed with another Group A10 service.
Once per occasion of service per distinct location. First patient at the location only.

Telehealth

10945·Telehealth Attendance – Support (<15 min)

Fee $40.00·85% $34.00

Professional attendance of less than 15 minutes by an optometrist providing clinical support to a patient participating in a videoconference consultation with an ophthalmologist — where the patient is in a telehealth-eligible area (≥15km by road from the specialist) or is a patient of an ACCHO.

Patient must be in a telehealth-eligible area OR an ACCHO patient. Videoconference with an ophthalmologist. Duration <15 minutes.

10946·Telehealth Attendance – Support (≥15 min)

Fee $79.80·85% $67.85

Professional attendance of at least 15 minutes (whether or not continuous) by an optometrist providing clinical support to a patient participating in a videoconference consultation with an ophthalmologist — where the patient is in a telehealth-eligible area (≥15km from the specialist) or is an ACCHO patient.

Patient must be in a telehealth-eligible area OR an ACCHO patient. Videoconference with an ophthalmologist. Duration ≥15 minutes.
